We Make Getting a CGM Easy and Fast

Our friendly and compassionate team handles all the hard work of getting a CGM.  Once you give us your details, your insurance and doctor’s information, we get to work!  These are the steps for getting a CGM we take care of for you:

  1. We confirm your CGM eligibility and insurance coverage with SCAN Health.
  2. Smiles contacts you doctor’s office to get the prescription and other required paperwork completed.
  3. After successfully completing the other steps, we ship your new CGM reader and 3-month supply of sensors to your front door using free 2nd day air delivery. 
  4. Every 3 months we will send you 3 more months of sensors to your door for free.  
 

Do I Qualify for a CGM (Continuous Glucose Monitor) with SCAN Health?

To qualify for a CGM from Scan Health Plans you must meet criteria 1 & 2 and meet at least one (1) of the items from criteria 3.

  1. You are Diagnosed with Diabetes Mellitus:   Your doctor has to have diagnosed you with either type 1, type 1.5 or type 2 diabetes.  

  2. Visit Your Doctor Every 6 Months: You must visit your doctor every 6 months regarding your diabetes.

  3. Insulin Treated: You must have a prescription for insulin noted in your doctor’s file for you.  An emergency use insulin pen would qualify as insulin treated.  Long-Term use of insulin is NOT required, only an insulin prescription in your medications list.  

    Experienced (2+) Hypoglycemic Level. 2 Events:  Patient experienced two or more (2+) blood sugar readings below 54 mg/dL. OR documented symptoms such as dizziness, sleepiness, weakness, irritability, trouble speaking or confusion. Experienced (1+) Hypoglycemic Level. 3 Events:  Patient experienced one or more (1+) level 2 event(s) AND a 3rd party was required to stabilize the patient. OR patient experienced severe symptoms such as; seizures, convulsions, loss of consciousness or coma.

  • Do I Need Pre-Authorization for a CGM from Scan Health
    • No – Scan does not require a pre-authorization for a CGM
  • Where Can I view SCAN Health’s CGM Requirements:  
  • Why is my patient’s CGM rejecting/not covered at the pharmacy?
    • CGM supplies are covered under the durable medical equipment (DME) benefit under Medicare. For SCAN members, DME supplies are provided by the medical group’s contracted DME supplier and are not coverable under the pharmacy benefit.
  • My patient previously received a fill at the pharmacy, why is their CGM refill now rejecting?
    • Your patient may have received a temporary fill during their first 90 days of enrollment with SCAN at the pharmacy. Known as a transition fill, this allows patients to have access to these CGM supplies while they are new to the plan. This transition fill also allows you time to request authorization (referral) from the medical group to cover and supply CGMs under the DME benefit.
  • Why are CGMs covered under the DME benefit and not the pharmacy benefit (Part D)?
    • Medicare covers CGMs under the DME benefit. For SCAN members, this benefit is managed by the medical group and will not be covered at the pharmacy. Please make sure you DO NOT send an authorization request to Express Scripts. They only manage pharmacy authorizations and will not process your request.
